Sports Physical Form Deadline: the Window Parents Miss

A sports physical has two deadlines: the day the form is due at school, and the day the exam stops counting. Here is how to work out both and book the appointment in time.

The sports physical form deadline is really two dates. The first is the day the school needs the signed form, usually before the first practice or tryout. The second is the day the exam stops counting, often 365 days from the exam date. Book the appointment 6 to 8 weeks before the season so there is time for both.

Most pages about sports physicals are written for schools chasing missing forms, or for clinics booking appointments. The parent side is simpler and easier to get wrong: you need the exam early enough, and you need to know when it stops counting.

When is the sports physical form due?

Your school or league sets the due date, and it usually falls before the first practice or tryout. A student without a valid form on file usually cannot practice. Look for the date in the athletics page, the coach's first email or the registration platform the school uses, and write it down the day you see it.

How early should I book the sports physical?

Six to eight weeks before the season starts. HealthyChildren.org, the parent site of the American Academy of Pediatrics, says the AAP recommends making the appointment at least 6 to 8 weeks before the season, so there is time for any extra evaluation or treatment. KidsHealth gives a similar rule: at least 6 weeks before.

That lead time matters more than it looks. Pediatric offices fill up in late summer, and if the doctor wants a follow-up, a two-week gap is not enough. The AAP also advises against mass physicals in a school gym or urgent care, because those settings often lack privacy and do not know your child's history. So the usual route is your child's own doctor, and that needs booking ahead.

How long is a sports physical good for?

It depends on your state athletic association, and the rule is written on the form itself. Two examples:

The Florida EL2 form prints the rule at the top of each page: valid for 365 calendar days from the date of the exam. Virginia's form says it expires 14 months from the date the practitioner signs it. Other states set their own rules, and some only require a new exam in certain years. Read the top of your state's form, or ask the athletic office.

Why does a physical expire before the season?

Because the clock starts on the exam date, not the school year. Say your child had a physical on February 20, 2026, for spring track. Under a 365-day rule it runs out around February 20, 2027, about a week before spring tryouts on March 1. Fall 2026 was covered. Spring 2027 is not, and nobody sends a reminder.

The fix is to write the expiry date down the day of the exam, next to the date you need to book the next one.

What if the doctor wants a follow-up?

This is the reason for the 6 to 8 week lead. Sometimes the exam turns up something that needs a second look: a heart murmur, a past concussion, asthma that needs a new plan, an old injury. The doctor may ask for a test, a specialist visit or a few weeks of treatment before signing. With two months of room, that fits before the first practice. With two weeks, your child may sit out the start of the season while the paperwork catches up. Ask at the appointment whether anything is pending, and write that date down too.

What should I check on the form before I hand it in?

Forms come back from the doctor's office incomplete more often than you would think. Florida's EL2 says a form is not valid unless all sections are complete. Before it goes in the backpack, check:

  • the exam date is filled in, and it is the date you expect;
  • the provider signed and stamped every page that needs it;
  • the parent and student sections are signed;
  • every page is there, in the version your state uses this year;
  • you kept a photo or a copy of the whole thing.

Is a sports physical good for one year?
