FSA Debit Card Receipt Request: Documents and Substantiation Deadline
An FSA debit card receipt request needs an itemized receipt or EOB for the flagged transaction. Follow the deadline in your notice, then check that the request is resolved.

An FSA debit card receipt request means you need to document a particular purchase, even though the payment went through. Send the itemized receipt or EOB requested, follow the response deadline in your notice, and check that the request is resolved.
Find the transaction before looking for a receipt
Open the request beside your FSA account. Match the merchant, amount and transaction date. Then identify the appointment or purchase behind that payment. A card charge posted after a visit may have a different date from the service itself; keep both dates in your notes.
Write a short reference line: provider, patient, service date, amount and request number. If the notice lists several transactions, give each its own line. One dental statement might explain several charges, while a single card payment might cover more than one service. Do not assume one uploaded file answers every request on the screen.
The payment receipt you found in your wallet may show only a total and the last digits of a card. Before sending it, look for what the administrator actually needs to verify. This check takes less effort than waiting for another request because the first attachment was incomplete.
What an FSA debit card receipt request needs
Employee Benefits Corporation's documentation guidance asks for an itemized receipt, provider statement or invoice, or an insurance Explanation of Benefits. It lists the service date, service description, provider and amount. EBC says a credit card receipt alone lacks the service information it needs.
Use this preparation checklist, then compare it with your own request:
- Match the document to the transaction being questioned.
- Check the provider and patient information the notice requires.
- Locate the service or item description, service date and amount.
- Make sure all relevant pages are readable, including any insurance adjustment.
- Keep the request number with the file you plan to submit.
An EOB and a payment slip answer different questions. Read the EOB's service lines and patient responsibility rather than grabbing the largest number on the page. If it does not clearly match the charged amount, ask the administrator which additional document would explain the difference. Do not change the document to make the numbers match.
The substantiation deadline comes from the request
There is no single response window that you can safely apply to every FSA card. EBC currently asks for documentation or repayment within 28 days of receiving its request. Benetech's substantiation instructions ask participants to respond within 30 days. Those are two administrators' instructions, not competing descriptions of one nationwide deadline.
Record the response date printed in your notice. If it gives a number of days rather than a date, ask which event starts that period and whether the submission must be received by a particular time. Use the earlier of your own planned work date and the official deadline to leave room for a missing statement.
Keep this task separate from the last day to incur eligible expenses and the plan's run-out period for filing claims. December 31 does not replace a response deadline that arrives sooner. A request about an old transaction also does not, by itself, tell you how much unused money you have left to spend.
Ask for the missing page, not just “a receipt”
If your document is incomplete, contact the provider's billing office with a precise request. Give the service date and explain that you need an itemized statement showing the services and amount to answer an FSA card documentation request. Add the patient name and any other fields your administrator has asked for through the provider's normal secure process.
When insurance processed the visit, check whether the insurer has issued an EOB for that service. A statement sent before insurance finished may not explain the amount now charged to you. Ask which version the administrator wants instead of submitting a collection of unrelated pages and hoping one works.
If the deadline is close, contact the FSA administrator while you wait for the provider. Explain what you have requested and ask how to proceed. Keep the answer with the notice. An unanswered extension request is not an extension, and a successful payment does not settle whether the expense qualifies.
Submit against the right request and check the result
Use the submission method named in your notice. Before uploading, open the file yourself: can you read the date, description and amount without zooming into a blurry photograph? Check that a multipage PDF contains the page with the service details, not just the account summary.
Save the submission confirmation with the original request. Note the transaction it covers and any review time the administrator provides. If several requests were open, check their individual status afterward. A receipt showing that a file arrived is useful evidence, but it does not necessarily mean the administrator accepted it.
For help keeping the follow-up visible, save a copy of the request yourself in dEssence and ask Tracy to identify the stated deadline and missing paperwork. You can ask for a reminder to send the documents and another to check the status. Use only the information needed for that task; Tracy does not decide eligibility, submit the FSA response or read your benefits account.
If the administrator rejects the attachment, ask which field or transaction remains unresolved. Keep its explanation, obtain the requested correction and follow its instructions. Avoid filing a second reimbursement claim for a purchase already paid with the FSA card just to clear the documentation task.
FAQ
Why do I need a receipt after using my FSA card?
The card payment may have gone through without enough information to verify the expense. The request identifies the documentation needed for that transaction; it does not automatically mean the purchase was ineligible.
Is a credit card receipt enough for FSA substantiation?
Often it is not. EBC specifically requires service details that a payment slip usually lacks. Check your administrator's requested fields before uploading the document.
Do I always have 90 days to answer an FSA receipt request?
No. Follow your notice. The EBC and Benetech examples above use different, shorter response periods, so a deadline borrowed from another plan can be misleading.
What should I do if I lost the itemized receipt?
Ask the provider for a replacement or ask your administrator whether the insurer's EOB will satisfy the request. If time is running out, contact the administrator before the stated deadline and keep its instructions.
